Important Things to Know
- Lake Park SD Cafeteria plan year is September 1, 2015 through August 31, 2016.
- MGM is the Third Party Administrator (TPA) for LPSD for plan year September 1, 20152 through August 31, 2016. Debit Card is available only to those schools who adopted to participate in the plan. Direct deposit is available.
- The dental provider is changing to Guardian, effective September 1, 2015. For new plan information, click on Benefit Highlights tab / Dental.
- Unum Disability: Guarantee issue amounts are subject to pre-existing condition limitations.
- Fort Dearborn is the new basic and voluntary term life provider to Lake Park SD employees. They are offering a guarantee issue enrollment this year! For new plan rates, click on Benefit Highlights tab / Life.
- Plan offerings to Lake Park SD employees for 2015-2016 plan year include: Guardian Dental Plan, Spectera Vision, Unum Educator Plan Disability, Loyal American Cancer and Accident, Fort Dearborn Term Life and AD&D and Flexible Spending Accounts.

The Flexible Reimbursement Accounts can be a valuable part of your benefits package that can save you a significant amount of tax dollars on out of pocket medical expenses you incur throughout the year. But if you have any excess funds in them at the end of the year, you will lose those extra dollars. This feature of the web site allows you to log in and actually view the account balances, what claims have been paid, and the amount remaining in your accounts allowing you to better manage the benefit reducing the risk of losing money at the end of the year.Retirement Planning
We have created another web site specifically tailored to assist employees in making sound financial decisions whether it is planning for a new house, saving for your children's education, or making sure the years after your retirement is as golden as you deserve. This site is intended to be an educational resource by including several financial calculators, general information on the different investment options available, and will even let you create a portfolio that will track returns. There is absolutely no sales from this site and no banners to clutter up its purpose of being an educational resource for you.Contact Us
We realize that people who work in the educational fields do not have an abundance of time to make calls to handle a benefit issue during the hours provided in a work day. This feature allows you to contact us at any time that is convenient for you. When you submit the contact form, it is automatically routed to the appropriate person minimizing the time it takes to get an answer back. We even provide space for you to describe the problem in as much detail as you wish on the form.Benefit Enrollment
During the open enrollment period this button allows employees to log in to the enrollment system. The system walks you through your personal information such as address and date of birth and allows you to update it as necessary. It also gives you a listing of the dependents that you have in your household so that you may elect coverage on them as well. It will show your current benefits and allow you to add, edit, or drop coverages. After the open enrollment period has ended, the functionality of this link changes to allow you to log in and view the benefits you are currently enrolled in. The ability to add, change, or drop benefits is disabled.
