Medications

  • Prescription medication may be administered at school, if the medication is sent in the original, properly labeled container and if the medication is for that particular student. In addition, written permission from the parent/guardian is required for the administration of medication at school. Students may not self-administer medication unless it is their asthma inhaler or emergency injectable epinephrin medication and they have provided the proper forms and information from their doctor to the campus nurse.
Sample medication dispensed by your physician may be administered at school if it is accompanied by written authorization from the doctor, as well as written permission from the parent/ guardian. The medication must be in the original, properly labeled container.
Over the counter medication may be administered at school if it is accompanied by a written authorization from the doctor, as well as written permission from the parent/guardian. The medication must be in the original, properly labeled container.

Please see the Health Forms page for the proper form that must be submitted before any medication will be administered to you child.

    Medication Policy

    • It is best if all medication can be given at home, before or after school.
    • If it is necessary to take medication at school, it will be stored in the nurse's office, and given at the time designated by the physician.
    • Students are not allowed to carry medication in backpacks, lunch boxes or keep it in desks or lockers.
    • The EXCEPTIONS are inhalers, insulin, epi-pens, and some eye drops, which responsible students are permitted to carry, after turning the self-administration medication form into the nurse.
    • The physician may FAX the medication order to the school nurse at (814) 689-3962.
    • A new medication permission form is needed for every NEW SCHOOL YEAR.
    • It is best if parents bring the medication and permission form to the nurse's office.
    • All medication must be in the ORIGINAL container, with the proper LABEL from the pharmacy, and within the expiration date, (no envelopes or baggies please!)
    • Please notify the nurse if your child is leaving the building for a field trip and needs to take their medication from the Nurse's office on the trip.
    • A teacher on the trip can assist with administration as needed.
