Lake Park School District Volunteer Program

PLEASE RAISE YOUR HAND TO HELP by volunteering with the Lake Park School District. We want YOU to share in giving back to our amazing community. Together we all achieve more.

A tier system has been developed based on levels of supervision.

Tier I

Parent with teacher supervision fill out Tier I Volunteer Information Form & Confidentiality Requirement Form

Tier II

Parent/Non-Parent without teacher supervision fill out Volunteer Information Form, Universal Screen Form, and Confidentiality Requirement Form

Tier III

Volunteer Coach/Non-Parent or Parent supervising over night trips, or activities without teacher supervision fill out Volunteer Information Form, Confidentiality Requirement Form, Background Check Release, and Fingerprints.

As a part of the application process, the attached forms must be completed prior to volunteering:

1. Volunteer Application;
2. Confidential Staff Emergency Profile;
3. Confidentiality Form;
4. Required Forms (fingerprints and criminal history)

The assignment you have requested requires fingerprint clearance in accordance with PARS 15-512. Please contact the District Office to schedule an appointment.  Fill out all of the forms, sign them and bring them with you to your scheduled appointment.

After Human Resources processes your application, the school will be notified that you are eligible to begin. You must wait for approval before beginning your assignment. The school will then contact you with your assignment and schedule.
